THCS – Transforming Health and Care Systems

The European Partnership on Transforming Health and Care Systems (THCS), co-funded under Horizon Europe and coordinated by the Italian Ministry of Health, supports transnational research and innovation aimed at making health and care systems more sustainable, resilient, innovative, inclusive and person-centred. This includes several initiatives exploring how health and care systems can better respond to population ageing and support healthy, active and independent living.

At the Festival, THCS will be joined in the exhibition area by four funded projects developing innovative solutions for older people and their care pathways:

• EMA_FRAILTY – Ecological Monitoring for the Assessment of Frailty in Older People
• STRIDE-COM – Smart Technology for Remote Intervention in Diabetic Foot Ulcer Prevention in Community Settings
• MI-RICORDO – Transcultural and Multidimensional Validation of Digital Rehabilitation Intervention of Cognitive Resources, Domain-Oriented
• TransCare – New Care Pathways for Supporting Transitional Care from Hospitals to Home Using AI and Personalised Digital Assistance Together

These projects address complementary aspects of ageing and care, from frailty assessment and the prevention of diabetic foot complications to digital cognitive rehabilitation and more effective transitions from hospital to home.

Visitors will have the opportunity to meet the project teams and discover how their solutions can contribute to more preventive, personalised and integrated models of care. Representatives of the four projects will also take part in the panel discussion “THCS in Action: Transforming Ageing Care Pathways Through Innovative Solutions”, organised by THCS on 14 September 2026 at 17:00 in Hall A. The session will explore how THCS-funded projects are turning research into practical solutions with the potential to transform health and care pathways and improve the lives of older people.

Le Partenariat européen pour la transformation des systèmes de santé et de soins (THCS), cofinancé dans le cadre d’Horizon Europe et coordonné par le ministère italien de la Santé, soutient la recherche et l’innovation transnationales visant à rendre les systèmes de santé et de soins plus durables, résilients, innovants, inclusifs et centrés sur la personne. Cela inclut plusieurs initiatives explorant la manière dont les systèmes de santé et de soins peuvent mieux répondre au vieillissement de la population et favoriser un mode de vie sain, actif et autonome.

Lors du Festival, le THCS sera accompagné, dans l’espace d’exposition, de quatre projets financés qui développent des solutions innovantes pour les personnes âgées et leurs parcours de soins :

• EMA_FRAILTY – Surveillance écologique pour l’évaluation de la fragilité chez les personnes âgées
• STRIDE-COM – Technologie intelligente pour l’intervention à distance dans la prévention des ulcères du pied diabétique en milieu communautaire
• MI-RICORDO – Validation transculturelle et multidimensionnelle d’une intervention numérique de rééducation des ressources cognitives, orientée par domaine
• TransCare – Nouveaux parcours de soins pour accompagner la transition de l’hôpital vers le domicile grâce à l’IA et à l’assistance numérique personnalisée

Des représentants des quatre projets participeront également à la table ronde intitulée « THCS en action : transformer les parcours de soins aux personnes âgées grâce à des solutions innovantes », organisée par THCS le 14 septembre 2026 à 17 h dans le hall A. Cette session examinera comment les projets financés par THCS transforment la recherche en solutions concrètes susceptibles de transformer les parcours de santé et de soins et d’améliorer la vie des personnes âgées.
