At this booth, we’re presenting our response to a silent crisis: at home, malnutrition and sarcopenia creep in unnoticed between doctor’s visits—and pave the way for falls, hospitalizations, and loss of independence. Our approach: detect these conditions before they become critical, through primary prevention. The clinical tool for independent dietitians: NutriRepère™.

A SaaS solution and web application that streamlines the identification of malnutrition and sarcopenia, in compliance with the HAS 2021, EWGSOP2 2019, and ICOPE (Decree No. 2026-191) guidelines, and that tracks each patient’s progress over time to prioritize high-risk cases. Accessible both in the clinic and at the patient’s home, with no installation required. The clinical decision remains the practitioner’s: the tool provides insight; it never makes decisions on their behalf. Continuous self-monitoring: SYNApps™. The secure social network for caregiving, available as a freemium service directly on a smartphone.

Between visits, the individual or their caregiver records daily observations: appetite, meal content, weight, hydration, fatigue, mobility, and isolation. The individual remains in control of their care journey—their data belongs to them. And the caregiver finds real support in this: their mental load is reduced. At-Home Nutritional Support: Équilibre™. The Accès Liberté™ plan dedicated to nutrition: daily support coordinated by a Care Manager, in collaboration with the independent dietitian who monitors the individual—eligible for the Personal Services tax credit.
