Summary of the application :
Communit-Action is an integrated community health programme to prevent frailty, reduce isolation and promote active ageing through social prescribing.
It connects healthcare, municipalities, citizens, third sector and businesses to deliver local actions such as adapted physical activity, walking groups, memory gyms, community nursing clinics and caregiver education.
