A propos de cette table ronde :<\/h3>\n\n\n\n

R\u00e9pondre aux d\u00e9fis du vieillissement de la population n\u00e9cessite de faire \u00e9voluer les syst\u00e8mes de sant\u00e9 et d’accompagnement vers des approches plus pr\u00e9ventives, personnalis\u00e9es et int\u00e9gr\u00e9es. Le Partenariat europ\u00e9en pour la transformation des syst\u00e8mes de sant\u00e9 et de soins (THCS), cofinanc\u00e9 par le programme Horizon Europe et coordonn\u00e9 par le minist\u00e8re italien de la Sant\u00e9, soutient des projets de recherche et d’innovation transnationaux visant \u00e0 transformer les syst\u00e8mes de sant\u00e9 et d’accompagnement, notamment dans le domaine du bien-vieillir.<\/p>\n\n\n\n

<\/p>\n\n\n\n

Cette session mettra en lumi\u00e8re quatre projets financ\u00e9s par le THCS, d\u00e9velopp\u00e9s pour am\u00e9liorer les parcours de sant\u00e9 et accompagner les personnes \u00e2g\u00e9es dans le maintien de leur sant\u00e9, de leur autonomie et de leur ind\u00e9pendance. De la pr\u00e9vention de la fragilit\u00e9 et du soutien \u00e0 la mobilit\u00e9 \u00e0 l’aide cognitive et \u00e0 la transformation des parcours de soins, ces initiatives montrent comment la recherche et l’innovation peuvent contribuer \u00e0 la mise en place de syst\u00e8mes de sant\u00e9 et de soins plus int\u00e9gr\u00e9s, plus durables et centr\u00e9s sur la personne.<\/p>\n\n\n\n

About this panel discussion :<\/h3>\n\n\n\n

Addressing the challenges of an ageing population requires health and care systems to evolve towards more preventive, personalised and integrated approaches. The European Partnership on Transforming Health and Care Systems (THCS), co-funded under Horizon Europe\u2019s Health cluster and coordinated by the Italian Ministry of Health, supports transnational research and innovation projects across different areas of health and care systems transformation, including many initiatives focused on healthy ageing and the needs of older people.<\/p>\n\n\n\n

<\/p>\n\n\n\n

This session will showcase four THCS-funded projects designed to improve care pathways and support older people in maintaining their health, independence and autonomy. From frailty prevention and mobility support to cognitive assistance and the transformation of care pathways, these initiatives demonstrate how research and innovation can contribute to building more integrated, sustainable and person-centred health and care systems.<\/p>\n\n\n\n
